A startup looking to solve the challenge of finding patient care after a hospitalization has just gotten a major vote of confidence from investors, including a prominent San Francisco venture firm.

Boston startup Careport Health, an alum of the Techstars Boston Fall 2012 class, has raised $3.8 million in funding led by Baseline Ventures, a San Francisco firm whose past investments have included Twitter, Instagram and ExactTarget.

Other investors in the round included CommonAngels Ventures, Excelerate Health Ventures, Generator Ventures, Launch Capital, 500 Startups and local investor/entrepreneur Andy Palmer. Prior investors in the company have included Boston Seed Capital and, of course, Techstars.

Careport’s founder is Dr. Lissy Hu, a graduate of both Harvard Medical School and Harvard Business School.

Using data on health care organizations, CarePort aims to match patients seeking post-hospital care with the high-quality and ideal options for their needs, “in an easy-to-understand and engaging format,” the company said in a news release. Patients at Massachusetts General Hospital, for instance, can use CarePort to find rehab options after an elective procedure, the company said.
